All indemnity insurance plans are welcome.

We are preferred providers for the following PPO Insurance Plans:

4.    United Concordia (Military Dependents)

5.    Humana PPO

6.    Ameritas

7.    Connection Dental

8.    Guardian

9.    Tricare (Delta for retired Military)

10.  Aetna PPO

11.  CHIP (Delta Dental)

We will be happy to file your insurance claims for you, but we do expect

your estimated portion to be paid at the time of service.

We DO NOT accept any HMO's or discount dental plans.
